Quantum Health Pilates & Yoga Lounge (QHPYL) Waiver

General Waiver and Release of Liability

In consideration of being allowed to participate in the fitness classes and activities offered by Quantum Health Pilates and Yoga Lounge (the "Studio"), I, the undersigned, hereby understand and agree to the following:

  1. Risks. I understand that the Activity involves risks of injury, including but not limited to (list of risks).

  2. Release of Liability. I hereby release and forever discharge QHPYL, its officers, employees, agents, instructors, and representatives (the “Released Parties”) from any and all liability, claims, demands, losses, or damages arising out of or in any way connected with my participation in the Activity.

  3. Assumption of Risk. I assume full responsibility for any and all risks, injuries, or damages, known or unknown, which I may incur as a result of my participation in the Activity.

  4. Medical Conditions. I represent that I am in good health and physical condition and have no injuries or medical conditions that would prevent me from safely participating in the Activity.

  5. Medical Treatment. I authorize (Quantum Health Pilates and Yoga Lounge) to seek emergency medical attention for me in the event of an injury or illness.

  6. Binding Agreement. This Agreement shall be binding upon and inure to the benefit of the parties hereto and their respective heirs, successors, and assigns.

  7. Governing Law. This Agreement shall be governed by and construed in accordance with the laws of the State of Washington.

  8. Entire Agreement. This Agreement constitutes the entire agreement between the parties with respect to the subject matter hereof and supersedes all prior or contemporaneous communications, representations, or agreements.

  9. Severability. If any provision of this Agreement is held to be invalid or unenforceable, such provision shall be struck, and the remaining provisions shall remain in full force and effect.

  • No-Show + Late Cancellation Policy 

  • Payment Authorization & Auto-deduct Cancellation

  • Photo Release and Use of Likeness Please let the staff of QHPYL know if you do not wish to have your picture/video made public.

QHPYL Photo Release and Use of Likeness Waiver               

By signing up for classes or events, I grant Quantum Health Pilates and Yoga Lounge (QHPYL) permission to use my likeness in a photograph and/or video in any and all publications and materials without payment or consideration made to me. 

I realize these photos and/or videos become the property of QHPYL and will not be returned.

I authorize QHPYL to use, edit, copy, publish or exhibit any photo or video for any lawful purpose.  I understand that I waive any right to review any photo or video or obtain royalties from any photo or video.

Please note: Photos and videos will be taken throughout the course of the day, and some photographs/videos will be posted on our website and newsletter. 

Please let the staff of QHPYL know if you do not wish to have your picture/video made public.

QHPYL No-show + Cancellation Policy 

For all classes Requiring Pre-registration:

All no-show or canceled classes within 10 hours will result in a late cancellation fee:

  • Pilates Reformer or Barre/Reformer w/ recurring monthly autopay membership = $15 fee

  • TRX w/ recurring monthly autopay membership = $10 fee

  • If you have a punch card, you will lose a “punch”.

If you are not signed in for class 2 minutes before its scheduled start time, you are considered a no-show and friends from the waitlist may be able to claim your spot.

All other classes (yoga, HIIT, etc.) that do not require pre-registration:

All no-show or canceled classes within 1 hour will result in a late cancellation fee of $5.

These fees are charged automatically by our booking software. If you feel that any charge was made in error, please contact us. 

Thank you for canceling on time so we can free up spots for others who wish to join!

QHPYL Payment Authorization & Auto-deduct Cancellation         

  • I understand and agree to be automatically enrolled in QHPYL auto-deduct program.

  • I understand and agree that I will be charged the monthly rate regardless of my attendance at the studio; whether my schedule or living situation changes.

  • I understand and agree that if my schedule or living situation changes, I may opt to freeze the auto-deduct for up to 1 month per year with a 30-day written notice delivered electronically.

  • I understand and agree that should I choose to cancel after my commitment period (completion of 3 months), that I will give a 30-day written notice delivered electronically. Notice received less than 30 days will result in that month being assessed, with the cancellation effective the 1st day of the following month.

A no-show or cancellation within two hours of any scheduled class will result in a $10 fee or if you have a punch card, you will lose a “punch”.
